Before your first visit you'll need to fill out the forms below and either email or fax them to us or bring them in during the night of your study:
In Lab Sleep Testing
Our sleep centers are set up like a hotel where you'll have your own private bedroom with satellite TV. We have designed them to make sleep testing as easy as possible for the patient.
When coming in for a sleep study you will need to bring these items:
- New patient forms
- Photo ID and insurance card
- Comfortable, loose fitting sleeping clothes
- List of current medications
- Make sure to have clean dry hair
In Home Sleep Testing
In addition to sleep testing in the lab we also offer in home sleep testing for the patients who prefer to be tested in the comfort of their own home.
If you are getting an in home sleep test, you'll need to have these items available:
- New patient forms
- Photo ID and insurance card
- Comfortable, loose fitting sleeping clothes
- List of current medications
- Make sure to have clean dry hair
